What Are My Breast Augmentation Options?

If you are eligible for breast augmentation, our surgeon will explain the options available and recommend those that can best deliver the results you desire.

Breast Implant Size

There are numerous implant size options, depending on the breast size and shape you desire. Implant sizes vary from 100cc to over 1000cc. The choice of implant volume is based on many factors including patient wishes, starting breast size, desired breast size, breast width, breast tissue elasticity, and nipple position. Our goal is to customize a treatment plan to meet your aesthetic goal.

Breast Implant Material, Texture, and Shape

There are two types of FDA approved implants in the United States: saline and silicone. Both of these implants have a common silicone shell.

The implant shell comes in a smooth or a textures form. A smooth texture is used more often because of certain safety features that accompany a smooth shell.

Implant shapes also come in round and anatomic shapes. The anatomic breast implants only come with a textured shell, so a round shape is utilized more often in our practice.

Breast Implant Placement

Your breast implants can be placed in two locations:

  • Below the glandular tissue but above the muscle (subglandular)
  • Under both the glandular tissue and muscle (sub-muscular)

The decision on placement is determined on multiple factors such as desired goal, starting breast size and elasticity, upper pole fullness or need for a lift.

Placing implants below the muscle is more common, especially in thin individuals and those who desire a more natural breast augmentation. Individuals who are extremely active, such as body builders, may be better served with implants above the muscle to minimize risk of animation deformity of muscle activity.

Breast Implant Incision

There are several incision options for your breast augmentation, and is typically surgeon dependent.

  • Periareolar incisions are made around the bottom half of the areolas and can accommodate small to moderately-sized breast implants.
  • Inframammary incisions are made along the natural crease under the breasts and can accommodate most breast implant sizes.

If breast augmentation is combined with a breast lift, then additional incisions may be required to achieve the most aesthetic result.

Adding a Breast Lift (Mastopexy)

Women who want to increase their breast size and are experiencing sagging due to pregnancy, nursing, weight fluctuations, or age may need to combine their breast augmentation with a breast lift to ensure ideal results.

Mastopexy lifts and tightens the breasts while raising the nipples to a more youthful position and also reduces the size of the areola.

Fat Grafting to the Breasts

For women looking to achieve a natural augmentation to their breasts without implants, fat grafting can be an exciting option. This technique transfers fat harvested from areas of excess throughout the body into the breasts. An average result is an increase of a half to a full bra cup. This option provides natural fullness and does not produce an augmented look, as can be achieved with an implant. It can also be combined with implants to help provide additional symmetry to the breasts, allow for more narrow cleavage or even help open up constricted breasts.

Best Candidates for Fat Grafting to the Breasts

  • Those who do not want implants or are ready to quit implants
  • Have suitable donor sites for fat removal
  • Desire a natural result
  • Desire a cup size increase
  • Have a normal mammogram (if older than 40 years of age)

About the Procedure

Fat is removed through liposuction from areas of excess. It is not uncommon to use multiple donor sites, especially for thin women. Donor sites can include the abdomen, flanks, thighs, arms, and back. The fat is cleaned, drained of excess fluid, and carefully transferred into your breasts using small two millimeter incisions at your breast fold and areola borders.

A typical fat grafting session can transfer from 250 to 500cc into each breast. The final volume is dependent on what the breast can accept and is patient specific. When you wake up from anesthesia your breasts will be very full and appear augmented. Expect about half of the volume to absorb over the next six weeks. A typical fat grafting procedure has an average take of 40 to 60% of the transferred volume. For example, if 300cc was transferred into each breast, expect about 150cc to take.

You can expect bruising and swelling of your breasts after the procedure but the same can be expected from the liposuction donor sites. You will be instructed to wear compression of your liposuction sites and receive lymphatic massage for up to three months. This will ensure smooth and clean liposuction results.

There is not much to do for the breasts while you are healing other than gentle support and limiting excessive pressure. It is important to ensure adequate nutrition — with a high protein diet — to allow for appropriate healing and good fat take.

The recovery and healing time is longer with fat grafting than with an implant; however, the results are likely to be permanent and do not require implant monitoring. For those interested in a larger, natural result, a second fat grafting session can be performed, at minimum six months later.

How Is Breast Augmentation Performed?

After general anesthesia is administered, the following steps will be taken:

  1. The agreed-upon incision type (inframammary, periareolar) is made. Based on your body type, lifestyle, desired results, and treatment plan, the pocket is created above or below the chest muscle to allow a hand in glove fit of your breast implants.
  2. Silicone implants are placed prefilled into these pockets, while saline implants involve having their empty silicone shells placed and filled to the desired size by our surgeon and sealed.
  3. Our surgeons close the incision, meticulously and methodically, to allow the best scar possible, once healed.

If you require a breast lift with your augmentation, the loose tissue is tightened and shaped around the implants. The areolas and nipples can be reduced and raised to a higher position when necessary, and excess breast skin is removed before the incisions are closed.

What Is the Recovery From Breast Augmentation Like?

The recovery from breast augmentation is not typically difficult, and downtime may range from five to 10 days, after which most patients can return to sedentary jobs. You should continue wearing your compression or support bra, and avoid strenuous activity, including lifting, for up to six weeks or until cleared by your surgeon.

Submuscular implant placement may slightly increase the intensity and duration of breast augmentation recovery.

Make your downtime more comfortable by following your post-operative instructions and these tips:

  • Arrange for assistance for the first 24 hours after surgery.
  • Pre-cook protein-rich meals that can be reheated and eaten without much effort, and drink plenty of water.
  • Prepare a comfortable place to rest with everything you need easily reachable, including an extra-long phone charging cord.
  • Take your medication as prescribed.
  • Do not lift anything weighing over 10 pounds.
  • Avoid submerging your incisions in water until cleared to do so.
  • Call the post-op or office phone number immediately if you experience signs of infection or severe pain.

What Results Can I Expect From My Breast Augmentation?

You will observe increased fullness in your breasts immediately after surgery; however, results take time as the implants settle. If implants are placed under the muscle it is not uncommon for the implants to “ride high” after placement. Settling of the implants occurs over the following two to three months, as the muscle relaxes and the implants drop into place. It can take up to a year for the final results.

Breast augmentation scars fade well and blend into the surrounding skin, making them less visible. Our team also uses a proprietary scar cream, silicone taping, and other optional treatments to expedite healing and promote faster scar fading.

Can I breastfeed after breast augmentation?

Women who choose to augment their breasts before having children often wonder if their surgery will impact their ability to breastfeed. Most often, women with implants have no problem breastfeeding, especially if their implants were placed through the inframammary fold and under the muscle.

Will breast implants raise my breasts?

While some women believe that adding implants to sagging breasts will help lift the breasts, implants do not affect droopiness. Instead, implants will only give you a larger version of what you already have. If you are struggling with sagging breasts, you will need to combine your augmentation with a breast lift.

Are my breast implants permanent?

Breast implants are not permanent devices, however, it is not uncommon for implants to last anywhere between 10 and 15 years. Some women experience issues sooner that require breast implant removal or revision, while others have their implants for much longer. Generally, when implants are removed or exchanged, it is typically at patient request and not because an implant problem exists.
